Summer Institute Forms
After you have applied to take the GIS 204 course in the ExplorEC Portal, you must also fill out the required forms below. The required forms and a $25.00 deposit (Check payable to UMM) must be received by the UMM Early College Office BEFORE you will be registered for GIS 204 and enrolled in the GIS Summer Institute. Forms can be downloaded and mailed to the address listed below. The UMM Early College office will also email all required and optional forms to the student and the parent where they can be filled out online through AdobeSign. Please choose whichever method works best for you.
Required Forms:
- Photo Release
- Health History
- Release and Assumption of Risk- under 18, age 18 and over
- Policies, Behavioral Expectations, and Travel Release

While there are numerous forms to complete prior to the student’s enrollment, they are all necessary to protect your child and to help make their Greenland Point experience the best possible. Remember when completing the medical form, that your child’s answers will NOT impact their ability to attend the GIS Institute. The more complete the information we have about a child, the better we may be able to meet their needs.
