{"id":561,"date":"2020-09-16T12:42:06","date_gmt":"2020-09-16T16:42:06","guid":{"rendered":"http:\/\/machias.edu\/registrar\/?page_id=561"},"modified":"2024-06-11T14:00:07","modified_gmt":"2024-06-11T18:00:07","slug":"updating-personal-information","status":"publish","type":"page","link":"https:\/\/machias.edu\/registrar\/homepage\/records\/updating-personal-information\/","title":{"rendered":"Updating Personal Information"},"content":{"rendered":"<ul>\n<li><strong><a href=\"#Official1\">Official Name Changes<\/a><\/strong><\/li>\n<li><strong><a href=\"#Preferred1\">Preferred\/Chosen Name<\/a><\/strong><\/li>\n<li><strong><a href=\"#Email1\">Email Account Name Change<\/a><\/strong><\/li>\n<li><strong><a href=\"#Address1\">Address\/Phone<\/a><\/strong><\/li>\n<li><strong><a href=\"#Gender1\">Gender<\/a><\/strong><\/li>\n<\/ul>\n<p><a name=\"Official1\"><\/a><br \/>\n<strong><u>Official Name Changes<\/u><\/strong><\/p>\n<p>A student who wishes to change his\/her legal name in UMS\u2019 Student Information System must provide a copy (via mail, fax or email) of one of the following documents showing the new name:<\/p>\n<ul>\n<li>Social Security card or Social Insurance Card (if previously on record)<\/li>\n<li>State driver\u2019s license or state issued photo ID<\/li>\n<li>Passport<\/li>\n<li>Military identification card<\/li>\n<li><a name=\"#2\"><\/a>Other legal name change documentation showing new name and former name<\/li>\n<\/ul>\n<p><em>Please note:\u00a0 Marriage certificates will not be accepted as name change documentation.<\/em><br \/>\n<a name=\"Preferred1\"><\/a><br \/>\n<strong><u>Preferred\/Chosen Name<\/u><\/strong><\/p>\n<p>The University recognizes that many members of its community use names other than their legal names.\u00a0 For some students and employees, a chosen or preferred name may be an important component of their identity.\u00a0 Therefore, the University has established this policy which allows students and employees to indicate their preferred or chosen names to the University community even if they have not changed their legal names.<\/p>\n<p><a href=\"https:\/\/www.maine.edu\/student-success\/name-usage-policy\/\">View UMS Name Usage Policy<\/a><\/p>\n<p><strong>To enter a Preferred\/Chosen Name in MaineStreet:<\/strong><\/p>\n<ul>\n<li>Go to <a href=\"http:\/\/mainestreet.maine.edu\">Mainestreet<\/a>.<\/li>\n<li>Navigate to your <em>Student Center<\/em>.<\/li>\n<li>Select the link labelled \u201c<em>Demographic Data<\/em>\u201d under \u201c<em>Personal Information<\/em>.\u201d<\/li>\n<li>Select the \u201c<em>Names<\/em>\u201d tab.<\/li>\n<li>Click the green \u201c<em>Edit<\/em>\u201d button.<\/li>\n<li>Add or change your Preferred\/Chosen Name.<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p><strong><u>Gender Identity and Pronouns<\/u><\/strong><\/p>\n<p>Students who wish to do so may enter gender identity and personal pronoun information in MaineStreet.
