UMM Student Alert
UMM Alert is a centralized referral system which captures faculty, staff, student, friends, and family members' concerns and incident reports. The information provided on the Student Alert form enables us to educationally intervene and to help students who may be experiencing academic or personal difficulties. The UMM Student Alert form is not a conduct form. Your referral helps us to intervene early on behalf of a student to help him/her be successful at UMM.
The Universal Incident Report form provides us with information to meet requirements of the Clery Act.
If you have a severe or life threatening medical emergency call 911. Then contact the Professional On-Call at 207-255-1209. After completing the form you will receive an e-mail confirming that your referral was received.

Use to report educational and personal concerns regarding a student (attendance, failing grades, lack of study skills, lack of direction/motivation, or other personal concerns).
- Universal Incident Report Form
Use to report accidents, potential crime, conduct infraction, or suspicious behavior in or around a campus building.
